As a Senior / Social Work Assistant, you will be responsible for the following duties:
- Perform assessment in financial application cases
- Assisting patients in national financial schemes application
- Assist medical social worker in reviewing assessing cases
- Work together with medical social worker in providing brief social assessment
- Collecting and organizing information for case assessment
- Perform home visits when necessary
- Educating patients and family members
- Act as mentors to new Social Work Assistant within the team
- Participate in quality improvement projects when necessary
- Any other duties within the team
Requirements:
- Min. Diploma in Social Work or equivalent
