Job Responsibilities:

1. Record Requests

a) Request all certified medical records in queue at that time and mail, fax or email to providers.

b) Complete all HIPAA authorizations that are forwarded with requests as well as Letters of Protection (LOP).

c) Communicate with medical facilities when a new client needs to be set up for treatment or exams, such X-ray, MRI, CT, etc., gather appointment date and time to note on the client’s chart.

d) Send referrals to third party to set up exams, ex. DMA or Access Health Management

e) Receives and requests checks for all medical record invoices for pending and received medical requests.

f) Request all cost checks daily for invoices received and mail them out.

g) Request all medical summaries and expenses from third party.

2. Received Records

a. Review records received to confirm records match what was ordered.

b. Records received are scanned, renamed, and e-mailed to assistant daily.

c. File all fax confirmations and distribute mail into proper electronic file.

3. Follow-Up

a. Contact providers daily from 7day follow-up list in access database to determine status of request. When provider confirms receipt of request, move to confirmed.

b. Resend any requests that were not received, correct any defective requests.

c. Once records are confirmed as received, check on status displayed on dashboard.

4. Canceled Requests

a. Call the providers to cancel and then sends a certified letter to them canceling the request. Scan into file and email assistant & attorney copy.

5. Invoices/ Payments

a. Scan all invoices for medical records after checks are requested.

b. Scan all check stub receipts into file.

Qualification and Experience:

-Must have a Bachelor’s Degree and practical work experience in record keeping and file management.

-Work Schedule- Monday to Friday at 9PM to 6AM

Statement of Service:

To request and follow-up on all medical records, bill retrieval, and lien requests for the firm in a timely and efficient manner.
